My elderly wife suffers from incontinence.
She was on medication which was updated.
However, She ended up taking the medication she had previously and the new one side by side. In effect, she was overdosing over the course of a month. Between the Incontinence Nurse, myself and the Pharmacist someone made a mistake. The Incontinence Nurse says the Pharmacist should have spotted that my wife should not have had both medications.
Who is at fault? Is there any point complaining? and to whom? My wife shows no obvious symptoms of any ill-effects. Is it a case of, as said in American football, 'no harm, no foul'?
