Combined Pill when BMI over 35

I figured I may as well post this here as many of us will have a BMI over 35!
Is anyone with a BMI over this (mine is 42) on a combined oral contraceptive pill?
I asked my GP to re-issue following a trial of Depo late last year and they are being difficult over it. I am aware of the increased risk of dvt and stroke, but is the only contraceptive I've had that hasn't given me any side effects and unfortunately I need to take a contraceptive as I have started on Isotretinoin for acne which you agree to do so when you start as it is teratogenic to the fetus. I'm happy to take the risk and have told the GP this.
Even more annoyingly I have taken it off and on for 10 years with absolutely no problem being issued and my BMI has been over 35 for the majority of that time.
They have agreed to a one month issue of the px whilst they confer with the Gynae Reg on the matter. But just out of curiosity, has anyone had this issue and over come it?

I moved to the mini pill due to high bmi
I then had the mirena coil

Taking combined contraceptives with a high bmi can be dangerous which is why they won't want to prescribe it

I'm happy to take the risk. I've had 25 years of trying other contraceptives and this is the only one to give me no side effects. I've still got zero sex drive following the Depo in Nov (and my regular 28 day periods have gone, not to return at all!). I don't even want my poor OH to kiss me. Which isn't like me at all.
I just want contraception that doesn't do awful things to me, and prevents pregnancy effectively! Combined pill is the only one to have done that.
