Passport forms

I am currently filling in my passport forms, and not sure whether to select renewal or changes to your existing passport as I am changing my name on it, but my passport ran out years ago.

Anyone know which one I should choose?

Just done mine, I am almost certain it's renewal but I left it for the post office lady to fill in!
