Isn't it just an injection into the hip area?

It's a vet holding up an old style glass thermometer to read the temperature after removing it from the horse.
Probably looks unfamiliar as it's rare for glass thermometers to be used any more - especially in equine practice.

Yes, it's a thermometer. From when they had mercury in and you had to shake them to get the mercury down, take the temperature and then try to read how far up the mercury had climbed.
