Support staff Occupational/ Health questionnaires

Hello, So I’ve been accepted into a new role for supprort staff in school. However I am being asked pre placement questionnaire by occupational health

I do not have any disabilities or conditions that require adjustments however I am being asked if I am waiting for treatments or investigations. I do see my doctor every 4/5 months and could potentially have surgery in 1years time. As this is not an urgent matter I am very lost in what to answer for this question.

I do not want to answer no, but I do not want to get asked questions in a years time reasons to my surgery
Does answering Yes mean I am not considered for the new role ?
Help please

I can only answer for my own local authority but as recruiting manager I don't see the content of health questionnaires. I am only told that the candidate is fit or not fit for the role or if there are any adjustments required.
There is no way that a job offer would be withdrawn on the basis that a candidate might need surgery in the future.
So I think you should answer truthfully. It's possible that there might be a follow up phone call to discuss your questionnaire so answer your phone if you get any calls from unknown numbers. Lots of people don't and that causes delays in the recruitment process.
