Third eyelid - any advice

For the last 10 days my two year old cat has a very obvious third eye lid protrusion. It covers half his left eye and quarter of his right (so worse in the left). Does he need the vet? I have no idea what it could mean having owned cats my whole life I've never seen this before!

He's ok in himself, not going out as much as he normally does but I've put that down to the sub zero temperatures!

Does he get into scraps outside by the way?

No he doesn't, he's a timid little thing and has never scrapped as far as I know. He tends to be a runner rather than an attacker!

Oddly they seem much better tonight, I'll see how they are in the morning. Thanks for your reply.
