I haven't, but my best friend is emigrating. She is using a company called Pet Air. It's expensive but they handle everything, including the import permit and quarantine in the destination country. They advise on rabies and other required vaccinations and provide the right sized crate for the flight.

I considered it but he's just not got the acceleration at take off.
