Stupid question from somebody who doesn't normally watch Dr. Who but is curious to know...

...what happened.

I once saw that episode where John Simms was the baddie, the Doctor turned very old and all the little thingies fell down to earth, with the Doctor's companion off to save the world.

What happened next? Did she manage it, and how? And where is she now?

Yes she managed it, by telling all the world about the Doctor and that they had to think of him at a specific date adn time. At that date and time the Doc then tuned himself into the psychic network (?) and came back to his strong youthful self. Defeated the lovely John Simms and went back to earth.
Where the companion (Martha) decided to no longer travel with the Doc but to stay home instead.

Yes, the whole world said the word 'DOCTOR' at exactly the same time, and it recharged his batteries, so to speak. She travelled the world, spreading the gospel. It was all quite biblical, really.

The thingies are called toclofanes

Martha saved the world by telling everyone to think of the doctor at the same time, (timed by a countdown the doctor didn't actually know was going to happen when Martha left him, but that's splitting hairs) and their thoughts were amplified by the satellite system the master (John Simms) had set up to control the earth in the first place, allowing the Doctor to break free & regain his youth. Captain Jack (John Barrownman) then destroyed the paradox machine which the master had made from the Doctor's TARDIS, thus turning back time to the point just before the invaders arrived (but after the American president had been assassinated)
Martha thendecided to spend time with her family (who had been kidnapped & held hostage by the Master & were pretty traumatised)and left the Doctor after telling him she wasn't going to spend the rest of her life waiting for him to notice her. But she left her mobile with him & told him to come if she called. She's since been in Torchwood & is coming back at some point in series 4.

So how did the doctor know there was going to be a countdown when he told Martha to use it to time the think-in? I don't remember the Master revealing his plans (beyond a little general run-of-the-mill world domination ranting) BEFORE Martha left?

Simple answer, he didn't. But us manic retconners suggest that..erm.. the Doctor knows the Master so well, he was able to accurately predict the irresistable allure of a countdown, and how long it would take to prepare his plans.

And then we slink off because that's a bit crap.
