Where are all the drs in Grey Sloan

Why are there no medical drs in Grey Sloan? They are all surgeons. Don't medical drs usually diagnose the patient and then call the surgeons if they need surgery rather than surgeons turning up to see if it's a surgical case? I've seen other drs such as obgyn or dermatologists but what about general medical drs? Why no medical drs in the pit? Only surgeons. They don't even seem to be friends with other drs. Or have a consult where a dr explains that their patient might need surgery.
They even seem to treat people with colds and ultrasounds. Why would a surgeon intern be doing an ultrasound on a pregnant woman?
Is it an American thing?

The show was originally about surgical residents and their journey. It’s always been focused on surgeons and occasionally ER/clinics rather than the medical side.

And the nurses appear to be invisible and not do much nursing.

I don't know the answer but I've also noticed that they refer to the chief as the chief of surgery and the chief of the hospital. Well which is it? It's like surgery is the only thing their hospital is used for!

in Grey Sloane the surgeons seem to treat the medical patients / and find an excuse to operate on them 😂
if I get admitted to hospital and need a physician, don’t give me the bloody surgeon please!

I understand that. And it should focus on the surgeons. But surely their paths would cross with other drs. And when depicting surgeons jobs they are definitely doing stuff that should be done by other medical drs. Eg tending to a person in the pit with a cold or victims of smoke inhalation, things that wouldn't necessarily fall under a surgeons remit.
