I mean…it starts off with child sex abuse and then moves on to a woman who has a butthole for a mouth

I haven't seen it but I know Edouardo Casanova as he is a visual artist I follow. When you mentioned "butthole for a mouth" I knew it was him.
His work is generally more of an art performance and a visual experience rather than a movie with a narrative which makes sense.
