Should probably start this by saying that Gomorrah- a series based on organised crime in todays Naples - is gritty, gory and not for the easily offended.
If you saw The Sopranos I'd say that it was a Gomorrah-lite. So if The Sopranos or even Game of Thrones shocked you then Gomorrah is probably not for you.

We're on episode 5, I think this season a return to form much better than season 3, which I didn't think was as good .
Episode 4 was set in London and, I thought, had some glaring holes in the plot. They seemed more relevant as I know London and have some idea of how the Police and criminals operate in England. It's like they tried to turn London into Naples.
Aside from episode 4 I think it's been an excellent series so far, in particular Patrizia is well written and Cristiana Dell'Anna plays the part brilliantly.
