I think Ann's eye rolling is a form of self editing, it prevents her saying things she wants to say, but thinks better of saying out loud.

I don't get the whole Courtney is a drag act but not addressing Courtney as Courtney is disrespectful. I understand Ann saying she's no more likely to address Shane as Courtney than she is to call Amanda, Alma (if Amanda appeared as Alma in her company) Usually actors are a bit at being addressed by character names, particularly if you know them personally, which I think is the distinction here.
