teen anxiety at school

my 15 yo dd is doing the absolute minimum for school (always has) while still managing to get reasonably good grades. I can remind her days on end to study or to prepare, but she'll ignore me. I feel i can't insist too much as her grades are indeed fine. I just find it a pity that she doesn't push harder, as she probably would be top of the class - however, I'm always carefull not to instill perfectionists dreams in her.
The problem however is that about once every two weeks she'll text me from school saying that she is super anxious about a test. It is not before every test, so it is not a major pattern. She feels like she will vomit or have diarrhea which I think is mostly caused by stress as she's not sick after the test. She's then panicking, not wanting to leave the toilets and only after much coaxing (over texts!) I manage to get her to go to the class room.
I've given her books/apps on mediation/breathing exercises but she thinks this won't help (I think she hasn't really tried). Is this something someone recognises? Have you found something that helps?
It could be some type of performance anxiety, but why (oh why) is she then not studying more beforehand? At home she's super calm about her tests and convinced she'll be fine without much preparation (which is in fact the case mostly). Tips most welcome
