They look like a risk assessment scoring system.

SGA= Small for Gestational Age i.e. a small baby
SFH= Symphysis Fundal Height- the size of your uterus when it is routinely measured with a tape measure (usually from 28 weeks). Factors that would affect that would be raised BMI, multiple pregnancy. I would imagine the risk scoring on these is to assess whether you need additional scans
PET= Pre-Eclampsia Toxaemia, more currently referred to as pre-eclampsia, a condition involving (amongst other things) raised blood pressure
Aspirin score= certain risk factors might mean you are advised to take Aspirin in pregnancy to reduce your risk of pre-eclampsia

Are you Surrogate Mother OP? Did the midwife not explain these notes? I'm guessing there is a score or the numbers are to do with levels of something, so you get a 1 not a 3 or a 5. Maybe 1 is good?
