Death after release from hospital

My friend lost her partner last year after he was admitted to hospital with an accidental drug overdose.

When the ambulance arrived at the house he was given an injection of adrenaline to bring him back to life, the paramedics at that time said that his heart would most likely stop again once this dose of adrenaline wore off and he would need another shot in hospital.

Instead of this happening, the hospital released him a few hours later and he died in his sleep.

Just wondering if anyone else has any experience with this or if any doctors can comment if this is normal practise.

My friends and her 2 young daughters are left devastated by this.
