who are highly recommended by several MNers, and also who I have visited in the past for various treatments.

I'm vaguely thinking a tiny bit of filler to counteract some sagging as I approach my 5th decade (naice way of saying I'm pushing 50 and haggard) and the nurse gave me her view that filler would give a good result, but for optimum results to have Thermage as well.

Has anyone had filler and thermage? or just thermage?

If you are pushing 50, then you are approaching your 6th decade not your fifth. Grin Sorry to break it to you, but am in the same (rather shabby) boat.

Have had filler- twice- but not Thermage.
I was told I didn't need it. But I did ask about my neck and was told that would need Botox and Dermaroller to get a 50% improvement and I declined- was going to cost over £1K and need constant top ups.

Have been very pleased with the fillers- mine were in my upper cheeks to avoid the cheeks sliding south look. Lasted over a year and am due to have again.

I had it done by Dr M, not a nurse.

it was actually more like, you can just have filler (small amount in 3 separate areas - I think she said it's called the Y area).
But ideally, she said, have thermage AS WELL.
I'm a bit unsure whether to have both or just filler. Obviously it would cost loads to have both but she did say the thermage can help the filler to last longer.
I wonder if I could request that Dr M does it.

You can ask for whoever you want- you're paying!
It's hard to advise- each person needs different things.

When I went for the consultation- not sure what you've done- I requested the appt with Dr M and paid £90.

I didn't mention money or my budget at all. I asked about some fillers to give me a lift - I had 2 horizontal creases on the top of each cheek and Dr M explained how this happens- it's the fat pads under the eye and top of cheeks separating. So the filler goes underneath ( not in the actual crease) to lift the area.
I had 1ml in each cheek and they charge by the ml.

I asked if I needed anything else and she said no.

The filler I had should last 9-12 months and I got 15 out of it. Now it's 15 since last treatment so due to go back.

You need to ask how long the Thermage + fillers would last and the total cost. You also need to know if you need a top up would they use the same amount of fillers or less. When I went for my 2nd lot, Dr M tried to get away with 0.5ml each side ( and it's half the cost of course) but my cheeks just gobbled it up.

I'd warn you that it is not pain free! I have a high pain threshold but found the fillers quite eye watering. You might need some pain killers for the day afterwards and your face might be quite sore for a few days- schedule the appt at least a week before you have any important meetings or events.
