How long has it been there?
Is he diabetic?

A scab maybe, but doesn't he remember how he got it? Any insect bites that he has picked at or something? If he can't remember and it doesn't drop off/heal in a few days then get to a gp or pharmacist to do a mole check on it - if it isn't actually a scab they may want to remove and biopsy it.

It depends.

How big is it?
How was he injured initially?
How long has it been there?
How old is he?
How's his circulation?
Does he smoke?
Is he a drinker?
Does he have diabetes of any type?

It may be an injury which isn't healing and may be the start of an ulcer due to poor circulation.

Get him to a GP. Sooner rather than later.
