It looks real to me. There's quite a hollow under your eye socket which could contain a cyst that size. Plus, we don't get a great view of the other side of his face in order to compare.
He could have had a local anaesthetic (I read somewhere that she was his girlfriend but was also a nurse) or he could have a high pain threshold!

He really should have seen a specialist though. Just too big and shouldn't be dealt with at home 🤮
