I’ve got some additional notes to add for the dla form and I’ve numbered them to match the question. Is that ok? Is there another way to present it? Do I just add these in the envelope? I’ve got 8 pages.
I have a lot of supporting evidence - about 61 pages. Do I just chuck them in the envelope? Or am I supposed to label them or write anything on the pages?

My deadline is 14th and I’m panicking it won’t even be delivered on time. So worried and stressed, all you parents are really just amazing for even completing the forms. I’ve found it really hard tbh!

Yes, it is OK to add extra pages for questions.

Put DC’s full name and NI no. at the top of each additional page and each page of the supporting evidence.

Send it tracked 24hr delivery. There is still time for that.

Her NI number is your payment reference number and labelling it like that will be fine at the top of the page. As in March 14th? If so you need to get it tracked 24 or if you can the special delivery by 1pm service in the post imminently and sadly there's a high chance now it won't get looked at in time so call them Monday to find out if it's been scanned on and to try to check they aren't going to stick it in the new claim pile.

The NINO is also the Child Reference Number (CRN). If this is a new claim and you don’t know it, put DOB.

If you are continuing an answer to a question on an additional sheet, say so on that question on the form. Number the additional sheets and on the form write something like “Please see additional sheet page 1”.

im really sorry but one more if anyone knows q.10 it says if you’re child is a Swiss national were they living in uk before year 21.
answers are yes, no, don’t know

my child is not Swiss national or even born before 21. Do I tick no or leave it blank?

Oh sorry it's a new claim, yes dob and their name is fine. I think it probably won't get backdated to when you called for it at this point but otherwise it's fine to get there past the date.

As long as the form is back with DWP by the 14th, which is still possible if you pay for one of the more expensive and quicker delivery options, it will be backdated. It doesn’t need to be scanned onto the system by the 14th, only delivered.

Full name and DOB is fine if you don’t know the national insurance number.

If DC isn’t a Swiss or EEA national, you don’t need to answer question 10.
