I have to fill in the sections on there 'About me etc' and I'm just blank ! 4 years of fighting for an assessment, we've got it and I don't know wtf I'm meant to put on the thing !! Ds is Autistic, high functioning (I know some people don't like that term but that's to give an idea) he attends mainstream and his main issue is he can't concentrate without an adult helping him

Do you meant the section about the child or about you as parent/carer?

in about the child I put a very brief history and then he filled in bits of good day/bad day/how supported

In the section on parent/caters aspirations I put what I wanted for my DC - as in good physical and mental health, and educational setting that supports and nourishes all aspects - then listed some stuff, achieve potential, self sufficient and self confident and a career, peer relationships and develop strats to manage challenges of his ND type stuff.

Oh we didn’t have anything as extensive as that on ours - just 3 questions.

Is your DC old enough/able to share his/her thoughts? Or do you have anything in an EP report where this has been explored.

on the 3 questions we had my DC just have some short answers - so for things that are important he probably would have said football and friends

Can't believe you only had 3 questions ! I hate stuff like this and I'm so desperate not to make any mistakes which might hinder getting the ehcp which he really needs

Some things to think about:

  • Things that are important to me - e.g. trains, lego, dog, special teddy, music, favourite football team, grandma, morning routine…
  • Things that are working well at home and school - what is going well and why e.g. quiet lunch club because it isn’t a busy and noisy, forest school because it meets sensory needs better than the classroom, nurture group because the group is smaller and better meets emotional needs, learning the drums, morning routine because it helps me transition from home to school…
  • Other information I think is important to know about me - e.g. any known triggers, best way to help DC regulate.
  • Important things to know about my past - e.g. anything important that has happened, prematurity/diagnoses/major surgery, siblings born, significant bereavements…
  • Things that I enjoy doing - what does DS like doing - e.g. art, lego, swimming, going to the park…
  • How to communicate with me - e.g. does DS have SM, communicate via AAC, lead people to want he wants, simple sentences, broken down into single points, ask for help when necessary…
  • My hopes, dreams and aspirations for the future - what does DS want to do? Train driver, work in a Lego shop, join a junior football club, learn French, learn to swim, go shopping independently, make dinner…
  • Things I would like to change at home and school - e.g. sleeping pattern, writing, assembly, sensory dysregulation…
  • Important things to know about our family history - e.g. any siblings, any family members with additional needs, parents now live separately…
