Who do you apply for EHCP etc to when you live in one count(r)y and DC school is in another?

This is possibly super identifying but hey ho.

We live about 100 yds from the border between 2 counties and 2 countries (both in the UK). For the sake of clarity we live in A but DS (7) goes to a school in B. It's the 2nd closest school to our house by road - the closest school is in our county but has no facilities to cope with a child with SN (30 pupils in whole school and only 2 staff members). Our GP is also in B because based on our postcode that's the country that the NHS considers us to live in, even though we pay council tax to A. Complicated huh.

So.... I want to put in a request for EHCP myself, but who do I request it from? The county where we live or the county where DS is actually at school?

The county where you live - you can name the school over the border of the county, if you think it can meet DC's needs, where the nearest one in your county can't.

Yes the county where you live,assuming the county where you live is in England. As I understand it there are other arrangements in Scotland and Wales.

Yes we live in England but almost all the services we use including schools are in the nearest town which is not in England!
