DLA renewal - lost copy of old form AAAAAARRRGGGGHH

Does anyone know if the DLA will send you a copy of the last form you filled in? I've just had the renewal form (DS2 is 5 in Oct) and I can't find the bloody photocopy anywhere!! Am dreading the thought of filling it all in again from scratch especially as I had a lot of really good advice about specific wording from my HV last time.

I think it's unlikely myself but do you think if I snivelled down the phone and explained I'd just moved house....?

For next time, type it out in Word then you'll always have a copy on disc. I never use the proper form any more.

They did for me. Phoned them up and asked for one, no probs.
