Changing SALT provider now an Academy: should I be worried?

DS goes to a great school which is in the process of becoming an Academy (no comment).
I got a letter in his bookbag today informing me that from Sept, they are changing SALT provider to a private company. DS receives SALT from school. Should I be worried about this? Is this fairly standard practice these days? Are there any questions/concerns I should be raising?

They've asked me to sign a consent form. I'm a bit uncomfortable with some random organisation being able to access and share my son's medical notes - or am I over-reacting?

It's a shame as he really likes his SALT lady. And as he has Autism, he doesn't deal with change at the best of times!
