ds seems to think that I am able to adequately clean his teeth through a hole just wide enough to poke a very small toothbrush through, then clamp around the stem of the brush.

Ds is a teenager and still does the whistle mouth thing! It has got better - gravity must have something to answer for - he can open up better for a short while when lying in bed- so we do at least get mostly one good brush a day. Thank goodness for electric brushes that can do the job faster!

You need a Collis Curve toothbrush. You really can clean all the teeth's surfaces by just shoving the toothbrush back and forwards in their mouth without all the tipping and angling you have to do with a traditional brush.
