How to write a letter for the Annual review and transition to secondary?

DD is in year 6 and has her annual review at the end of November. All letters of advice have to be into the school by the 14 November. We don't have a problem with anything in the statement and just need to put in writing what school we want to be named as her secondary school. I looked on IPSEA's website and copied a bit of their template letter. Does this sound ok?

Dear Sir or Madam,

X name, X DOB

I am writing as the parent of the above child who has a Statement of Special Needs and attends X School.

I am writing to request that the school that X will attend in September 2014 and to be named in the Statement is X School.

Sounds fine to me. You put in the school application that she has a statement, when you did your Secondary school selection?

It's a special school that we're wanting her to go to, so can't apply directly.

We had to go to Panel. I wrote a letter stating why it was necessary for him to attend and that was read out in the panel. He got in!
