funding for modifications to the house?

Is there government funding for minor alterations to your house needed for a disabled person´s benefit? I thought I´d heard about something along those lines, but can´t remember much about it.

We really need to change the entrance to our house, the door frame is very high and sits on top of a little pointless step, it´s all so difficult for dd2 to negotiate and she falls every other time. It means putting in a new front door and new flooring in the hall etc., so if funding was available it would be nice.

ah d*mn, through SS - might as well give up now. :( Our SS are spectacularly, outrageously useless.
Thanks for the info, Fio. I think I might get Carers Resource on board to lend me a bit of support in that case.
