at 12pm we have a app a the hosp for a dicharge care plan meeting, as my son has spent alot of time in hosp since he was born, he is 20 months old now and has 6 dx and is always in hospital, he was in 3 weeks ago for 11 days with chest infection then rotavirus
we have alot of medical staff coming to this meeting and im soo nervous
i have my list on what i wnat to discuss and i just hope they listen to me and can give us a good plan of action on how to help us and matthew when hes in hospital
does anyone have any last tips b4 we leave
