I'm filling in the CAF form with our school nurse this afternoon. She said if I had a look through it first it'd be helpful as it's quite a long form.
I'm just wondering what I should put in for the aims/actions section.
We are waiting for an appointment with CAHMS for him to be assessed.
His main issues are an inability to follow instructions, sit still and poor fine motor skills.
I know that anything I put should be SMART, but not sure how to begin...
