It has stared at me for about a week..
I have printed off the download one to make mistakes on rather than the one they sent me.
I have the cerebra booklet thingy.. And my diary.. And medical EP and other reports.
I have peeked inside...

Dh is supposed to be helping.. He has his nose stuck in an online game..grrrrr

I have a huge pot of coffee going and I'll be on here inbetween pages done.. Dh can then copy it all out onto the real form.. as you can actually read his hand writing.

dont do it!!

if you must do it slowly and take your time but remember you can always send more info in.
They turned us down straight and then when we ask for reconsideration they gave us HRC!!

Start by filling in the personal details, name, address, bank details.

Then do paed/SALT/EP/school

That way a lot of it is done before you know it.

Do you have to do personal care or mobility or both?

Yep, lost a load of it. Am ringing them tomorrow to verbally give answers. Fortunately it's generally Gp and consultant info not my written responses.

Ds2's renewal form arrived two weeks ago.

I've done bits of it but it's horrible.

When is the deadline for the form? Pace yourself, spread the questions over a few nights/weeks if you have to. As long as you get it in by the datestamp it'll be backdated to the day you requested the form.

I recommend writing up the answers in Word and just put 'see attached sheets' on the little boxes. That way you're not confined to the size of the boxes and can cut/paste.

Don't forget to put name/DOB on any extra sheets/reports you send in. And photocopy your answers before you send it so you've got a record for the renewal.

Blimey..okay.. I'm stopping for tonight got through ( in pencil on my draft copy) to childs mental health bit..
Great tip about using word and seperate sheets.. The bloody boxes aren't big enough!

Quick question .. lots of the how long do you do this for boxes.. Dhand I were looking dumbfounded at each other say said at the same time.. Flipping consantly!
So shall we just take waking hours, minus the time we don't do it.. And put that in?
Although showering was easy because we use a timer to do showering!

I will be going over to add lots more in over the coming evenings... And now I feel like pooo !

We are going through both the mobility and the care bit.. Intitially thought no to mobility.. But ds is dangerous if not with someone when out.. So as soon as I read.. " in a place they would not know" we both went " ahhhhhhhhhhh"

Ach. [feelingyourpain] I did it online and lost lots of it. Repeatedly. Berluddy thing. Downloaded it in the end, did it all in word on separate sheets. It's the way to go, definitely.

I sent it in about two weeks ago. In a large ring binder.

They called me today, rather agitated because I'd got my sort code wrong. That's probably a good sign, right?

Arsenic for those parts where I couldn't specify a time, I just put 'all day/continually/' and explained at 'other info' why I couldn't be more specific.

We got our renewal through yesterday. I still remember the horror of the month or so it took me to complete the form!

For anyone with a child with ASD, I found this really useful.

Iv stopped for the evening but at least I feel as ifi have made good headway.
Have gone over a couple of questions in good detail now.. Filled in all school and medical info and organised a friend who is an SN TA to do her bit :)

time for a horlicks I think .. And I have my bibic interview tomorrow for ds ( hoping we get some sensory stuff sorted out for him if we ever get there)

Go for it, girl!

Once you get started it just flows, but it can get quite wearing after a while so take plenty of breaks.

Give me a prod if you need a hand.
