Scoring for Bayley and sensory profile questionnaires.

Hi all,

I've been asked by the OT to fill out the above questionnaires which I have done. I know she will score it for me but in the meantime does anyone know how I can get hold of a score sheet?

Here is a sample report which shows the scoring technique.

The questions are grouped into subsections, scores in each section added, then compared to a table of values.

That is for the Winnie Dunn Sensory Profile.
