I cant find the DLA form online!!

Can anyone help me please?

I was told by SN HV that its on the direct gov website but can only find the DLA form for adults on there and I need the one for children...

I got loads of help from here for DD's

Fel1x, if you phone for a form, you get 6 weeks to fill it in, and if you return it in that time, they will backdate the award to the date of your phonecall.

If you do it online, they will only date it from the date you submit it.

Our SN HV is going to come round and help me fill it in. Not sure if they will award it to us.. DS has AS and HV says they are really strict so may not award if they think hes not bad enough so am going to need help to fill it in properly I think to amke sure I cover everything!
