how or who do i ask to see dd records from hospital?

i want to see all her records from our local hospital and a&e who do i ask and any body know how much?

when I got my maternity notes copied, I rang the PALs office in the hospital. They gave me the name of the person to speak to in medical records who sent me a form out. It cost me £25 for the notes for 5 babies so there was quite a bit of paperwork for that much. You can view them for free, also by going through medical records.

some info here it would seem they can charge up to £50 if they are paper records or £10 if computerised.
