Does anyone know of a proforma letter for requesting a statement?

My dd (12) was diagnosed last week as having Aspergers. I was advised to request a statement from the local education authority, as it was up to me to push this through.

I'm trying to find a link for a proforma letter requesting a statement. Can anyone help?

British Dyslexics do one for dyslexia which you could adapt:

IPSEA web site have proforms asking for educational statements.

Am I being very thick here, I can only find letters requesting assessment?

Dd has already been assessed and diagnosed, but this does not mean she will be statemented. The Ed.psych who diagnosed her recommended I write asking for a statement.

Sorry, I'm new to this and can't find what I need despite googling like mad

Peaceflower, your DD has been assessed by an Ed Psych. That is not the same thing at all as a Statutory Assessment.

A Statutory Assessment is a legal process, whereby a Local Authority assesses the educational needs of a specific child, by looking at all the available information from Medical services, Social services, the current educational setting, the Ed Psych, and any other organisation involved with the child such as CAMHS, dieticians, etc.

At the end of this statutory assessment, a decision is made as to whether a statement of special educational needs will be issued for the child.
