Annual review paperwork cock up

Had ds AR two weeks ago. We was told all year what a good year ds had had. The last AR had been horrendous due to a alfwul year. Ds has ASD and all the fun that comes with that.

The AR report was a week late to be distributed ( ie not within the two weeks legal deadline of the meeting).

It was very negative, said ds was aggressive, violent and had made no progress at all against of of his outcomes. None

I was really shocked because we had a great year, great behaviour logs, great parents evenings. Definitely no aggression or violence in behaviour log.

I quired why the reports was so at odds with everything I was told all year. I asked two school teachers, the senco and another SMT. No one could or would clarify how things were at odds.

We went to the AR a week after expressing our concern about the report and was told in the meeting the report was wrong. This is the first I knew of this.

In the meeting it was discovered more was wrong with the report as I was still querying it with the senco. Senco was denying it until the teacher said "I didn't write that!" Then only admitted the mistake when backed into cornor by overwhelming evidence of another mistake.

We walked out the meeting and a hour later got sent a new AR document. But with the same mistake in it. The senco apologised but said there was nothing they could do. It was partly written by a teacher who left in July 24 and that teacher had written ds progress for March 2025 in advance!

I looked harder at the report. I t has 6 outcomes on page 1 that the overall the years progress was marked against.

However the past years has been marked against five completely different outcomes that are totally different.

Last term only three outcomes had progress marked against them. The third target is completely missing ( no row in the report table)

Summer term completely missing

Spring 24 term.only had two outcomes for progress and both are recorded as progress made in March 2025. The third target again completely missing as was the fifth ( ie no row for them in the report table)

School keep on saying they don't understand what I'm talking about. Ds had been advised to go a sen School on the this utterly wrong report.

How do you mark progress against outcomes for another criteria that you recorded less than 50% of?

If you disagree with the paperwork, email the LA informing them.

I would also complain to the HT if the paperwork is inaccurate.

You have a right to a mainstream education unless a mainstream placement is incompatible with the efficient education of others, and no reasonable steps could be taken by the LA to avoid that.

If the LA amends the EHCP, you will get the chance to state your preferred school and that can be a mainstream if that is your preference.

Absolutely my child. Senco was defending its contents even in the face of evidence I had from behavioural logs. Child's name is in the provision too. Ie "Billy will...."

LA was in the meeting so they are aware but my LA are notoriously bad. I don't know if the school genuinely can't understand the issue or if the are playing dumb to gloss over their mistake.

Say target 1 is Billy will reduce learning gap but the target 1 he was monitored against in Spring and Autumn is about anxiety. They school look at me like I'm explaining quantum physics. So much is wrong with it. I honestly don't know now if I trust my child even needs the ehcp at this point because how can follow it when you didn't monitor the progress? Then confidential say no progress was made? Then they did make progress after all?

Even if the LA attended the AR, it is still important you email them challenging the content of the report that has been circulated following the AR.

It is important to remember the EHCP contains outcomes rather than targets. Outcomes are the result of provision, whereas targets are an aim. Schools may have targets on their internal IEP/school support plan but that is different.

Is this Secondary school? We once had a report as part of an AR that said “ Stephanie is very disruptive in music”
Not her name and she didn’t do music! Nor was she disruptive.

I’d ask for a meeting to unpick everything and highlight the points that are not accurate or misleading.
In future I’d ask for the meeting to be rearranged if you don’t have the paperwork in time.
Maybe take someone with you to the meeting from the Parent Partnership service.

If you don’t get answers and explanations from the SENCO or Head I’d contact the SEN Governor.
