Failure of access arrangements GCSE

As part of his EHCP our son has access arrangements in place for exams. This is extra time, a quiet space and a reader
he is in Y 10 and had a GCSE paper yesterday in ICT Media, it forms part of his final mark
school failed to put any adjustments in place, he took the exam in the main hall with everyone else and no additional time etc. he even asked if he was meant to be there
i am fuming, I contacted school and they are looking into it. Clearly things have gone badly wrong. The TA said that he has been allocated a separate room and couldn’t understand what had gone wrong, she has referred it up to the SLT.
what can I do, you can’t go back and do it again ? This is so unacceptable.
I mentioned the week before that access was not put in place for a maths test and was assured things would be ok for Monday.
for context he has asd , SPD and dyspraxia.

In my experience you have to be ‘that parent’ in the months leading up to exams. I have 2 doing A levels currently in 2 different schools both with complex access arrangements

we have been planning and organising for at least 6 months and have still had an issue twice with one child, school hate ringing me now because they know we insist on them actually doing things properly

I do know there have been problems getting enough invigilators but my DC is not going to be disadvantaged, it is the schools problem to sort

time to speak with the exam officer because they can apply to the exam board for an allowance as they didn’t follow his access arrangements I believe

thnx for the reply, I am so annoyed. The school called today and said sorry but sorry doesn’t cut it as you don’t get another go. They at least owned the error and are going to contact the board and ask for special consideration
I was reassured last week that access arrangements were in place. So disappointed for DS
