Worried for DD dyslexia/dyscalculia??

DD (5yrs in May)- concerns were raised by her teacher in April re her speech and language. Celf pre School 2 reported carried out within a fortnight. All tests were reported as being within average range- apart from Concepts and following directions which was slightly below (therapist agreed to reassess in 6 months but says it was the last test in a 40 min session and DD may have been tired??)
The school report flags up concerns about DD' finding organising her thoughts difficult' 'finds it difficult to apply phonics skills to reading and writing' and 'counting variable and difficult to assess because recently counted backwards in 10's from 100 yet sometimes struggles to recognise and count in 1s to 10'
I can see she has already lost a lot of confidence in all academic aspects of school already and am very keen to know how to help and/or what might be the problem. (DD excels at PE dance drama and has no problems socialising!!) I have not had formal meeting with SENCO yet just some suggestions of games that may help. SENCO thinks dyslexia and dyscalculia but says DD is too young to test? Any thoughts and or advice please?
