Help - can't cope anymore

My 15 month old was a great sleeper up until around 10 months, I'm a SAHM and he's had really bad separation anxiety, he goes to creche 4 afternoons a week for a few hours but even then took 2 months to settle him in. His sleep was manageable up until around 12 months. Before that he'd go to bed at 7 sleep until 7. Also my husband had a mental breakdown 3 months ago and has not really been home since (he went to stay with family then was hospitalised for 6 weeks due to return home for good next week - could this be having an impact?) The last few weeks it's been awful, my son resists bedtime, we have been battling for 2 hours this evening, the only way he will fall asleep and even then it's a battle is in my arms then I transfer him to cot. Last night he woke up at 2am wouldn't go back to sleep, in his bed or in my bed (most nights he ends up in my bed as its the only way I can sleep and even then its not a deep sleep but better than nothing) so I dragged my duvet into the lounge, curled up on the sofa and let him play. He did this until 6am when he looked tired then he slept for 2 hours curled up with me. This is why he has done every night this week. He's also very fussy with eating, refuses all milk, eats all his breakfast then nibbles lunch, eats his afternoon snack, refuses dinner...just screams when he's in the high chair.... Before 12 months he was a great eater. He naps for 1 nap of 2 hours per day. I'm at my wits end. What am I doing wrong? Is he over tired? Should I try an earlier bedtime than 7pm? Is he hungry is that why he wakes up, he will usually drink some milk but not alot in the night and I only offer it if he's not eaten much that day. Won't drink any milk in daytime, refuses all of it.
