Please help me formulate a plan to sort out DD's sleep. 16 months old

Occasionally DD 'sleeps through' more often than not she wakes up screaming at random times in the night and is up for the day around 5am. We can't take anymore. I think her problem is that she doesn't fall asleep on her own but goes mental if she's left alone. I've made a few half arsed attempts at gradual retreat but they've always been scuppered by illness or us going on holiday. I really can't take anymore and it's affecting the whole family plus my marriage as me and dh are just so grumpy and sleep deprived. I need a plan!!! Thank you 😊
