Any tips on breaking the patting to sleep habit?!

My dd2 is 8 months and will only go to sleep if patted. She sleeps on her tummy and I basically have to gently hold her in place while patting her bottom for at least 10 minutes. (She also is a nightmare at night but that's another story!)

I really want to teach her to go to sleep on her own for a number of reasons, one being that we have just lowered the cot mattress as she is now pulling herself up so it is killing my back! I have tried leaving her a few times but I always end up going back and patting her as she just goes crazy.
Now she has learnt to pull herself up so she stands in the cot and screams. I don't think she actually knows how to get back down!

Any experience or tips? My first wasn't a great sleeper but always fed to sleep or went down ok.

Ok I know some people find them freaky but a zaky (or the similar frog) is great for this
Dd used to like being patted so I would pat and over time I gradually slowed down the pats and held my hand on her longer. Eventually it was just hand on her which I would then exchange for the zaky
