16 wk old - no sleep day or night. Help!

OK, dd2 (16 weeks), has never slept well in the day. She's exclusively BF and a typical day generally ends up with her collapsing in a screaming, exhausted heap in mid to late afternoon. I used to be able to get her to nap in a sling but I am typing this with a wriggling bundle strapped to my chest and no sign she'll drop off soon (her 2 year old sister is fast asleep on the sofa!). She screams in the pram if we go out although she will nap in the car - but I can't afford to drive her around for hours on end.

Anyway, the last two nights she has also been wide awake from approx 1am to 4am, not crying but just gurgling and kicking in the cot. Both nights I have eventually managed to settle her by bringing her into bed with me.

Help! How do I get her to sleep? Is this the dreaded 16 week growth spurt? I feel particularly bad for dd1 who is being left for far too long on her own while I just try to keep dd2 quiet.
