We had this with DD2 as well. Teeth brushing was a nightmare too.

I think I saw something like this in Mothercare yesterday. Might have been a doctor though.

We have yet to take ds but have you tried any books? I remeber our dentist was really good giving stickers etc for being good, but you've probably been down that route already...

Oh, YG, have you seen those Playdoh dentists sets? That might be just the thing. You have a cartoon-y head and dentist's drills and stuff, and you make playdoh teeth and fillings and a tongue. I don't know if they are still available but it might be ideal. You could always get a doctor's coat from a dressing up set to go with it.

My kids had dress up clothes from asda.

Was pair of trousers and top in a blue/green colour and you had dentist/doctor/vet name labels that stuck on with velcro.

Also came with couple of tools and white mouth mask that hooks over ears.

If you don't have an asda and you want me to lookif they still have them just let me know.
