I second the advice to alternating hand and mouth quite frequently. Also try keeping your hand wrapped around the base most of the time, so you don't need to fit the whole thing in your mouth. That also minimises the risk of gagging.

I don't think there is a best position. I quite often find myself lying to one side, then move round a bit, then move again. I wouldn't mind finding a best position.

Agree with the advice of using your hand for at least 50% of the hobby, with lots of saliva.
