Advice on helping kids with separation

Has anyone managed to bring their kids relatively unscathed through a separation and got any advice? I’m frantic with worry about my 3.5 year old who, one year on, is still not coping and if anything is continuing to seem more anxious and clingy and generally regress. Not sure what we’ve done so wrong as we tried our best (have kept things structured and positive between us, we spend Friday afternoons together the three of us, our communication is better now than it was when we were together) or how we can help her now (have tried play therapy but she wouldn’t engage and it only seemed to upset her.
