Young woman sleeping rough

Hello all,
I have noticed for the second time now, on the pavements outside a London rail station a young woman around 19-20 sleeping rough. She looks in a bad way, not a 'functioning' homeless person if you understand me. Looks like she is going downhill fast.
How is the quickest way to get effective help to her?

This has some info on what to do and a contact number for the street outreach team if you are worried about a rough sleeper/homeless person.

I just realised, I donated to St Mungos in January, for a friends birthday but didn't know what it was. I will contact them now.
