Anyone had a mortgage approved recently for a house with spray foam insulation? What was required from bank to lend?

Normally they would require evidence that the spray foam has been completely removed.

It often can’t be removed without destroying the entire roof, so expect a complete re roof to be required before getting a mortgage,
