House on uneven ground - how have you extended?

From the front, our house is at normal ground level, but when you open the back door there are steps leading down to the garden. There are perhaps 2 foot of foundations are exposed at the back of the house. The steps from our back door lead to a (deathtrap) patio and there is a further 1ft drop from patio to grass.

We want to have direct access from kitchen to the garden, but don't want to extend at the level our kitchen is at as we would tower over neighbouring gardens. We want privacy for us and for our neighbours.

Assuming an unlimited budget (I wish!) we'd be happy to knock the conservatory, patio and the two sheds.

So does anyone have experience of split level kitchens and extending out and down!...Really looking for inspiration and google is not being my friend!

I'm not good at this sort of stuff but could you put the steps inside your house, so steps down from the existing building to the extension then extend at the lower level?
