Loft conversion - permission to alter

My apologies if this has been covered elsewhere but I am hoping someone can help.
I own a 1st floor leasehold flat. It is a terraced house which has been divided into two so we only have one flat below us which is owned by our freeholder. When we purchased the flat we also had a lease amendment which included the loft for 5k.
My question is that we would now like to do a loft conversion. Do we need to ask the freeholder for permission to alter? Can she charge us to do so? I've been seeing horror stories about people being charged 10k + in order to do so but I can't figure out if that is because they didn't own their loft in the first place. As we do own the loft can she charge us? As we've already paid her 5k to have it included in the lease amendment then surely we shouldn't have to?
Any help would be much appreciated!
