Could anyone help me draft a letter of notice to my landlady?

Need to give it to her by Friday.

Writing letters/official stuff isn't where my talents lie.

Also, we're partly moving because of the state of disrepair (which she is aware of) the house is in. Should I mention that? Or just keep it naice and polite and short?

I don't think you need to actually tell her why you're moving.
