Experience at filling in CRB form needed please. I am being very dim today!

Right I am feeling very as have had my CRB form returned from school, (just for volunteer work)as I hadn't filled it in correctly. A loooooong time ago I was married before and therefore had a different name to now. On my application form there is only room for two different surnames.

Mine should read (if there was room)

birth - 1998 maidename
1998 - 2000 first married name
2000 - 2003 back to maidename
2003 - date crumpled name

Where is the room to put this extra information?

Realise I am probably being extremely dim and therefore should maybe not be allowed to listen to the little darlings read as can clearly not fill in a form properly!

Just filled one in.
i would suggest your current name in section A
Then your maiden name in section C under surname at birth andn then your old married name at line 22 nAny other surname used.
HTH it me ages to fill mine in - it is so fiddly.

Hi, I'm a CRB Recruiter (I sign off CRB ID) and I would suggest the following:-
Fill in C20 and 21 with your birth surname and when you used it until.
Then, I would tick H67 to say you are going to add additional information.
On a clean sheet of white paper, put your full name, current address and form reference (front page of the application form, right had side) at the top of the sheet. Then, write Additional Personal Details, Section C:-
then just list your dates and names as you have done above.
Honestly, that's all you need to do - don't worry about it! It's a shame that somebody at school couldn't have told you how to do this. I'll check back later, so please post if you need any extra information.

I'm going to get a new form from school today and fill it in tonight. Thanks so much, I expect they all think I'm a real scarlet woman now!

Oh and be sure and put any middle names too!

My application was returned as I didn't include a middle name that I've never ever used
