Worried for DD7

DD7 is in year 3. In our area children move schools to junior school at this point.
I have been so worried about DD7 since starting at junior school.
In the first couple of weeks the children were assessed and placed in sets for maths and reading. She left year 2 with a good report and on target in both areas but the junior school have placed her bottom across the board (they have made her aware of this) so she has begun saying 'I can't' and 'because I can't' at home. she's a quiet introverted child, we have tried so hard to boost her confidence, so this is such a shame.
I honestly thought her reading was good for her age (!). the school have put her into a group and shes bringing home reading books with only a few words on a page which she is rattling through. when i questioned this the school told me she is missing her phonics sounds. i have gone through all the sounds at home (provided by the school and again she just breezes through them.) its like we have a different child to the one that goes into school. :-(
in maths, i had no idea that she was already expected to do long addition/subtraction in infants year 2. i've now started practicing these with her at home to boost her confidence.
i keep thinking have i let DD down, is DD really far behind her peers? i wondered if others have struggled going from year 2 to 3? and any tips i might not have thought of to help my DD?

@Worrying1985 Have you never been given an overview of the national curriculum for each year at infants school? You sound like you are guessing at what the curriculum is. The reading books are very basic so I’m guessing she failed the phonics test. I think you need to talk to her teacher and work with them on homework to improve what she can do.
